Multicentre double-blind RCT (51 arginine / 47 placebo) in vasculogenic ED. IIEF-6 and cavernous peak systolic velocity both improved, but only 24% reached scores compatible with no ED — mostly the mild-ED subgroup. High daily gram burden vs citrulline trials.

Safety red flags
- On nitrates: cardiologist decision first; treat lightheadedness, meaning dizziness from a blood-pressure drop, as a signal to stop and call.
- With PDE5 inhibitors: prescriber approval first, with blood pressure monitoring.
Label reading notes
- Six grams per day is a large capsule/powder load — check serving count on the label.
- Low-dose "NO booster" arginine (1–2 g/day) sits far below the vasculogenic ED trial dose and will not match that evidence on the label.
- Do not stack with PDE5 inhibitors or nitrates without clinician sign-off. GI side effects are common at effective gram doses.
