IIEF-5 rose from 16.4 to 21.0 against no change on placebo, and 66.6% reported improved erection. Testosterone, prolactin and cholesterol were unchanged, so this is not a hormonal mechanism. Single centre, mild to mild-moderate ED only.

Trial floors
Primary
1000 mg three times daily (de Andrade 2007) · 3 g over 12 weeks · Benefit shown — IIEF-5 rose from 16.4 to 21.0 against no change on placebo, and 66.6% reported improved erection. Testosterone, prolactin and cholesterol were unchanged, so this is not a hormonal mechanism. Single centre, mild to mild-moderate ED only.
Other floors
900 mg three times daily (Hong 2002) · 2.7 g over 8 weeks · Benefit shown — Double-blind crossover with an 8-week washout design. IIEF total and RigiScan penile tip rigidity both improved; 60% reported better erections. Explicitly published as a preliminary report.
Cochrane pooled estimate (effect rated trivial) · 3 g · Primary endpoint missed — Nine RCTs, 587 men. Pooled IIEF-15 erectile-function difference was 3.52 points against a minimal clinically important difference of 4, and IIEF-5 was 2.39 points — Cochrane called the effect trivial at low-to-moderate certainty. The dose is real; the benefit at that dose is small on average.
Safety red flags
- On insulin or a sulfonylurea: tell your prescriber; watch for symptoms of low blood sugar.
- On warfarin, a DOAC, or antiplatelets: tell your clinic, and watch for bruising or bleeding.
Label reading notes
- Trial doses were 2.7-3 g of root per day split into three servings — many capsules hold 200-500 mg, so reaching the trial dose can mean 6-10 capsules daily.
- Korean red ginseng means steamed Panax ginseng. American ginseng (P. quinquefolius) and Siberian ginseng (Eleutherococcus) are different plants with different evidence.
- Extracts standardised to ginsenoside content are not interchangeable with whole-root grams; a blend listing 35 mg of ginsenosides is not a 3 g root dose.
- Do not stack with PDE5 inhibitors without clinician sign-off — both act on the nitric oxide pathway.
